name (if applicable), address, email address, & phone #. |

2. Please choose in image for the front
of your label; we have many images from which to choose (or attach
your own photograph, artwork, or logo as a jpeg). |

3. Please type in the message you would
like to have on the front part of the label. |
|
4. Please type in your wording for the
side panel of the label. |
|
5. Choose a type of wine and the number
of cases (mix up to 2 wines per case; minimum order is 1 case). |
|
6. A 50% deposit is required prior to
label design ; we will contact you for a credit card # and order
confirmation. |

design work, and will email you a proof for your approval prior to

print and bottle your wine in as little as two weeks!

number of cases (mix up to 2 wines per case; minimum order is one

the label. We have many images from which to choose, or you can send
us a photo, company logo, or other artwork. |

3. Write out the wording for the
front and side panels of the label. |
|
4. A 50% deposit is due prior to
label design. |

5. We'll do all of the computer
layout and fax, email, or mail you a proof prior to
printing. |

6. Please sign and fax back your
proof or email us your printing approval. We'll then print and
process your order. |
